I would say just ask her for her name, then talk to her for a bit, then ask for her phone number. If she gives it to you, then just call her up and ask her on a date.

first get her into some conversation,talk to her a bit then make her laugh by saying something funny or narrating some funny incidence then tell her she has beautiful eyes,then ask her "if you could have the honour to go out on a date with her". well thats what i have gathered after all my dates till now.

Just you need to start a conversation. If you can manage, it pretty easy to ask for the date then.
